All families will need to fully complete and send items 1, 2 and 3 in to the agency, along with a full payment for the services requested to begin their process.

IMPORTANT: Families who have lived in North Carolina for at least 5 years may request a criminal history record from NC SBI for a $14 fee. Other families will need the FBI ID Record Request for an $18 fee. Please check with your homestudy worker to make sure you have completed the correct checks.
